Working as an incorporated doctor (PJ) has become standard for shifts, clinics and hospital contracts. When properly structured, the legal entity reduces the tax burden compared with receiving income as an individual. But some details make all the difference.

Why so many doctors become PJ

Receiving income as an individual, the professional can reach the top bracket of the IR table. As a legal entity under Simples Nacional (simplified tax regime) or Lucro Presumido (presumed-profit regime), the effective tax on revenue tends to be much lower — which explains the migration.

Which tax regime to choose

For the incorporated doctor (PJ), the choice usually comes down to two regimes:

The fator r — the ratio between payroll and revenue — is decisive under Simples and must be calculated case by case.

The corporate structure

A doctor who practices alone typically uses the SLU (Single-Member Limited Liability Company), which protects personal assets without requiring a partner. In clinics with more professionals, the LTDA is the natural path.

Important precautions

  • Mandatory pró-labore: the working partner must have a defined pró-labore (owner’s salary), with the corresponding INSS.
  • Correct CNAE: medical activity has specific codes; the wrong one changes the taxation.
  • Service agreement: formalizes the relationship with hospitals and clinics and provides legal security.
  • Professional council: keep your professional registration in good standing.

Being a PJ does not eliminate obligations — it organizes them. The real gain comes from the right classification, not merely from “having a CNPJ (company tax ID).”

Is it worth it?

For most doctors with recurring income from shifts and contracts, yes. But the size of the gain depends on the regime, the fator r and how the pró-labore is designed.
